IV Therapy for Boston Marathon Runners: Pre-Race Prep and Recovery

Every third Monday in April, 30,000 runners come through Boston — and roughly 60,000 supporters come with them. Our nurse team has been serving Boston Marathon runners since 2018, and the same protocols we use for the elite masters field are the ones we book for weekend warriors from Newton, Wellesley, and Back Bay in the week around Patriots' Day.

Here's exactly how we use IV therapy for Boston Marathon prep and recovery.

The week before race day

The mistake most first-time Boston runners make is thinking they can carbo-load and gatorade their way to hydration on Sunday night. That is not how plasma volume works. Serious runners bank hydration in the 48–72 hours before the race, not the night before.

Two days out: The Rookie hydration drip

A liter of lactated Ringer's plus magnesium and B-complex. Not stimulating, not diuretic. It quietly rebuilds plasma volume without the 3 am bathroom trips that come from chugging water.

Race morning

Not the time for a full IV. Coffee, banana, a bagel, sip water. Save the drip for after.

Post-race recovery

The four hours after crossing the finish line at Boylston are the single most important recovery window. Muscle cells are inflamed, glycogen is depleted, and dehydration is real (marathoners typically finish 2–4% dehydrated even in mild conditions).

What we build for post-race patients:

  • 1L lactated Ringer's or normal saline (rehydration)
  • Magnesium (cramp prevention)
  • B-complex + amino acid blend (protein synthesis restart)
  • Glutathione or high-dose vitamin C (oxidative stress from a marathon is real)
  • Toradol IV if indicated (for musculoskeletal pain, per clinician approval)

The peptide question

Serious runners occasionally ask about peptides. We prescribe BPC-157 and TB-500 for legitimate soft-tissue injury recovery under medical supervision. These are not race-week medications — they are 8–12 week protocols. But if you finished Boston with an aggravated Achilles or IT band issue, book a consult with our peptide team.

The medical piece

Every marathon-related drip we run is administered by a Massachusetts-licensed RN and reviewed against your run history and any medications you're on. If you took NSAIDs during the race, Toradol is off the table. If you have kidney history, we adjust the fluid protocol. This is the reason to use a nurse-owned clinic and not a pop-up.
