Botox in Boston: Pricing, Units, and Choosing an Injector

Botox is the most-performed cosmetic procedure in the country, and in a city with as many medspas as Boston, the price spread is wide. Understanding how it is priced is most of what you need to shop intelligently.

Per-unit vs per-area pricing

Per-unit pricing charges for each unit of neurotoxin used. It is the more transparent model: you pay for exactly what goes in, and you can compare clinics directly. Per-area pricing quotes a flat fee for a region such as “forehead” or “crow's feet.” It sounds simpler, but the number of units inside that flat fee varies enormously between clinics — which is how an underdosed treatment ends up looking like a bargain until it wears off in eight weeks.

Ask any clinic quoting per-area how many units are included. If they will not say, that is your answer.

Typical units by area

  • Glabella (the “11s” between the brows) — commonly 20–25 units.
  • Forehead lines — commonly 10–20 units, and usually treated alongside the glabella to avoid brow heaviness.
  • Crow's feet — commonly 10–12 units per side.
  • Lip flip — 4–6 units.
  • Masseter (jaw slimming or clenching) — 20–30 units per side.

These are typical ranges, not prescriptions. Muscle mass, prior treatment history, and the look you want all shift the number.

Who should be injecting you

In Massachusetts, neurotoxin injection is a medical procedure. Look for a nurse practitioner, physician assistant, physician, or RN working under proper medical delegation — and specifically one who injects regularly. Facial anatomy is unforgiving; the difference between a natural result and a heavy brow is millimeters. Ask how long they have been injecting and ask to see before-and-after photos of faces that look like yours.

How long does Botox last?

Three to four months for most people. Frequent, intense expression and high metabolic rate shorten it; consistent treatment over time often lengthens it as the treated muscles soften. Results begin appearing around day 3 and settle fully by day 14 — do not judge the result before then.

Aftercare

Stay upright for four hours, skip strenuous exercise and hot yoga for 24 hours, do not rub or massage the treated areas, and avoid facials or lying face-down that first day. Bruising, if it happens, is minor and short-lived.
