Weight Loss Injections in Boston: 7 Questions to Ask Before You Start

Weight-loss clinics have exploded across Boston in the last two years — everywhere from medspas to online-only pharmacies now offer semaglutide or tirzepatide. Most patients do not realize the range in quality is enormous. A safe, medically supervised GLP-1 program looks nothing like a website that ships a vial with a QR code.

Before you start with any provider — including us — here are the seven questions to ask.

1. Who is prescribing, and can I meet them?

A licensed physician, nurse practitioner, or PA has to prescribe GLP-1 medications. Some online clinics use an out-of-state clinician who reviews a form and never speaks to you. At IV League Hydration, every patient meets our clinician on their first visit at our South Boston clinic, and our medical director Olivia Kelly, NP reviews the plan.

2. Is the medication FDA-approved or compounded?

There is a legitimate role for compounded semaglutide and tirzepatide — especially during shortages — but you should know which you are getting and why. Ask which 503A or 503B pharmacy the clinic uses, whether the pharmacy is state-licensed, and whether the base is the same active ingredient as the branded drug. If a clinic will not name the pharmacy, walk away.

3. What is included in the monthly price?

Boston pricing for weight-loss injections ranges from about $199 to $600+ per month. The gap is not random. Ask what is included:

  • Medication (which strength, which dose ladder)
  • Clinical visits (how often, in-person or telehealth)
  • Labs (baseline CBC, CMP, HbA1c, lipids, and follow-up)
  • Nurse messaging support between visits
  • Titration adjustments — free or extra?

A $199 program with no labs and no clinician access is not cheaper. It is a different product.

4. What labs will you run before I start?

Minimum baseline: CBC, CMP, HbA1c, lipid panel, TSH. Consider adding a lipase (pancreas baseline) for anyone with a history of GI issues or gallstones. If a clinic skips labs entirely, that is a red flag — GLP-1s are safe drugs, but not risk-free, and you deserve a baseline.

5. How do you handle side effects at 9 pm on a Sunday?

Nausea and constipation are common. Panic at 9 pm is common too. Ask: is there a nurse or clinician you can reach the same day, or is it a portal message that gets answered in 48 hours? 6. What is the plan when I hit my goal?

Most weight-loss clinics do not have a maintenance protocol. Patients hit their goal, stop the medication, and regain 30–50% within a year. A serious program plans your transition: a gradual dose reduction, a maintenance dose (often much lower), or an off-ramp with a structured food and training plan. 7. Where is the clinic physically located?

Online-only weight-loss clinics can be convenient — but if something goes wrong (severe nausea, an injection-site reaction, a suspected gallbladder issue), you want a real address you can walk into.
